Factor Associated with Symptomatic Recurrence of Rheumatic Fever and Rheumatic Heart Disease on Children Under 18 Years Old in Tangerang

Background
Rheumatic Fever (RF) and Rheumatic Heart Disease (RHD) remain significant causes of cardiovascular diseases in the world today, particularly in developing countries. The symptomatic recurrence of RF and RHD contributes to the worsening of rheumatic valve disease. However, there are few studies describing the factors associated with symptomatic recurrence of RF and RHD.
Purpose
To give an overview of the factor associated with symptomatic recurrence of RF and RHD.
Methods
We analyzed subjects with purposive sampling from medical records. There were 49 patients under 18 years old diagnosed with RF and RHD based on the Jones Criteria which was revised by WHO in 2003 and had received primary treatment and secondary prophylaxis, respectively. Bivariate analysis was utilized for comparison between two variables.
Results
Patients consisted of men (44.9%) and women (55.1%), which all patients were first diagnosed as RF (22.4%) and RHD (77.6%), whom age category of patients under 9 years old was 46.9%, and patients between 9-18 years old was 53.1%. Patients had variations of the first ASTO values, consisted of 200 IU/mL (46.7%), 400 IU/mL (42.2%), and 800 IU/mL (11.1%). Rheumatic carditis that was obtained was pericarditis (9.1%), myocarditis (22.7%), and endocarditis as the most (45.5%). We had found that 32.7% of patients had symptomatic recurrence (50% of men, 50% of women). Variables of RF and RHD diagnosis, sex, age category, and all types of rheumatic carditis were not associated with symptomatic recurrence of RF and RHD (each of which was p>0.05). The ASTO value in the first diagnosis was associated with symptomatic recurrence of RF and RHD (p=0.045).
Conclusion
The symptomatic recurrence of RF and RHD occurred in 34.7% of patients. The factor associated with symptomatic recurrence of RF and RHD is ASTO value in the first diagnosis.
